Placer County Aging and Disability Resource Connection (ADRC)(Emerging)
The vision to create the Placer County ADRC began In November, 2013, as the Placer County Older Adult Advisory Commission (OAAC) started discussing the Aging and Disability Resource Connection (ADRC) Model. They determined it would be beneficial and there was potential for implementation in Placer County. On September 15, 2015, the OAAC hosted a Community Forum on the ADRC Model, leading to formation of a broad-based ADRC Advisory Committee, which began meeting in March, 2016. Representation included local providers of long-term services and supports (LTSS), joined by Placer County Health and Human Services, Veterans Services, Hospitals, Managed Care providers, and consumers.
The Placer County Aging and Disability Resource Connection will provide a coordinated network of service delivery for the aging and disabled population and their circles of support.
The Placer County Aging and Disability Resource Connection will provide access to a coordinated network of services and supports using a “person-centered no wrong door” system, promoting and enhancing independent, safe and healthy living for our aging and disabled populations and their circles of support.
